I eat fast food and restaurant food 3-5 times per week. When I cook at home its a frozen dinner or a PB&J sandwich.

I eat a balanced diet of grains, protein, fruits, vegetables, and calcium-rich dairy. I am able to cook myself healthy meals and will occasionally eat out with friends/ family.

My kids (or friends) tell me I forgot things or that I am confused sometimes.
I have a degenerative physical disease/ disorder such as ALS, Parkinson's, history of stroke, etc. that makes it hard to complete activities of daily living.
